Your premium election divided by the current rates will provide how many hours are needed each month to maintain eligibility.
Your remaining coverage can be determined by dividing your dollar bank balance by your current monthly premium election. A participant’s dollar bank balance is only a bookkeeping account that is used to determine eligibility for coverage; a Participant has no vested right to the balance of their dollar bank.
Initial eligibility is met when your dollar bank meets the highest Trust PPO premium which is; employee, spouse ($35 surcharge), dependent ($25 surcharge), Time Loss Plus ($19 surcharge, $400 benefit). Employer contributions are reported to the Trust Office on a lag month schedule (i.e., hours worked in January are received in February to grant March eligibility). In this scenario, February is the lag month. Please reach out to the Trust Office at 800-872-8979 for the current premium rates.
If you become inactive for twelve consecutive months with no activity on your account, your Dollar Bank reserves will revert back to the Trust and you must re-establish initial eligibility.
Eligibility under the medical plan include coverage for both vision and prescription drug benefits.
The Trust offers a 24 hour nurse line. Please call 866.363.0950 anytime to receive assistance with any questions regarding your immediate health care needs. Registered nurses will answer your questions and refer you to a physician, minor emergency facility or emergency room if needed.
